Can You Take Oral GLP-1 with Other Medications? Drug Interaction Guide

If you take other daily medications, adding an oral GLP-1 raises timing and interaction questions that don't apply to the injectable version. Here's what the clinical data and prescribing information say.

FDA-Approved Oral Semaglutide (Wegovy/Rybelsus) Interactions

The biggest practical interaction with oral semaglutide is timing-based, not drug-based. The 30-minute empty-stomach requirement means you can't take other morning medications at the same time. Most providers recommend taking oral semaglutide first thing upon waking, then waiting 30 minutes before other medications, food, or beverages.

Oral semaglutide may slow gastric emptying, which can affect how quickly other oral medications are absorbed. This has been studied for common co-medications: levothyroxine (thyroid), lisinopril (blood pressure), warfarin (blood thinner), digoxin (heart), metformin (diabetes), and oral contraceptives. In most cases, clinically significant interactions were not observed, but individual variation exists.

Foundayo (Orforglipron) Interactions

Foundayo has no food or water restrictions, so timing with other medications is simpler. However, like all GLP-1 agonists, it slows gastric emptying. The prescribing information recommends awareness of this effect when co-administering drugs with narrow therapeutic windows.

Compounded Oral GLP-1 Interactions

Compounded oral GLP-1s haven't been formally studied for drug interactions. The active ingredient (semaglutide or tirzepatide) has the same interaction profile as the FDA-approved versions, but the specific delivery format (sublingual drops, gummies, etc.) may introduce additional timing considerations depending on the absorption pathway used.

Always tell your prescribing provider about every medication you take — prescription, OTC, and supplements. This is especially important for blood thinners, thyroid medication, diabetes drugs, and oral contraceptives.

Medications That Require Extra Caution

Insulin and sulfonylureas: GLP-1 agonists lower blood sugar. Combining with insulin or sulfonylureas increases hypoglycemia risk. Doses may need adjustment.

Other GLP-1 agonists: Don't take two GLP-1 medications simultaneously (e.g., oral semaglutide plus injectable tirzepatide). The Foundayo prescribing information specifically states that use with other GLP-1 receptor agonists is not recommended.

Oral contraceptives: GLP-1 medications may reduce absorption of oral birth control. Consider backup contraception during initial treatment and discuss with your provider.
